The root cause for acidity that is the release of excess acid is due to the interaction of histamine with receptors in the stomach walls.

Sodium hydrogen carbonate or magnesium or aluminium hydroxide work by neutralizing the excess hydrochloric acid present in the stomach, with out treating the root cause for release of excess of acid.

But cimetidine and ranitidine control the root cause of acidity due to which there is decrease in the acid released by stomach.

Hence cimetidine and ranitidine better antacids than sodium hydrogen carbonate or magnesium or aluminium hydroxide.

The main cause of acidity is excessive secretion of HCl in gastric juice and use of sodium hydrogen carbonate can make the stomach alkaline(basic) and trigger the more production of HCl. But cinetidine and renitidine can control the main cause of acidity that is production excessive HCl in stomachand relive the problem of acidity.Other antacids such as milk of magnesia,lansoprizol,omeprizol etc. can also be used for problem of acidity.

Excessive use of sodium bicarbonate or a mixture of aluminum and magnesium hydroxide can make the stomach alkaline and trigger the production of even more acid. On the other hand, rantidine and cimetidine prevent the interaction of histamine with the receptors present in the stomach wall. This results in release of lesser amount of acid. Thus, these are better antacids.
