Conductivity: Represents the amount of heat that can be conduct by material

Conductance: It is reciprocal of resistance, it basically involves the conductivity, shape and area of the geometry.

Conductivity relates to the material only while conductance relates to conductivity as well as shape and area.

Hope it helps
