Normal vector to plane is
(3-1) i + (4-2) j + (5-3) k
= 2i + 2j + 2k
now
equation of plane will be like
2x + 2y + 2 z = d
now it passes through (2,3,4)
hence
4 + 6 + 8 = d
d = 18
hence equation of plane is
x + y + z = 9
