polyp and medusae are the two forms. polyp is sessile and cylindrical  shaped and medusae is free swimming and umbrella shaped.
example of polyp – all members of class anthozoa
example of medusae- all members of class scyphozoa
 
members of claa hydrozoa undergo metagenesis i.e poly produce medusae asexually and medusae produce polyp sexually
