No. of double bonds are 5 ( 2 amide, 1 secondary amine, 1 keto, 1 ether, 1carboxyl )
Degree of unsaturation is no. of double bonds + no. Of ring = 3+3=6
Therefore answer is (4)

The number of Functional groups present are 5 : -
 O              O

C  —OH , C —NH2 (2), —NH , —O— 
Now there are 3 double bonds , 3 rings , 1 triple bond which means 3+3+2 = 8 double bond equivalent/degree of unsaturation.
Note: DBE / DOU of 1 double bond = 1
           DBE /DOU of 1 ring =1 
           DBE/DOU of 1 triple bond =2
