Simple answer in two words “muscle fatigue”.

A slow injection requires the doctor to adduct the thumb for a longer time and apply constant downward pressure with the tip.. It might be possible steadily for approximately 40–50 seconds before fatigue sets in, and you get cramping out your hand starts to wobble and shake and the pressure applied also varies. The bore size of the needle is constant and the material is unyielding and cant be influenced by most external factors, however.. Here too .. You have laws of physics like the Hagen–Poiseuille law operating.

Doctors nowadays prefer using syringe pumps for doing the work of the doctors hand while administering an injection which requires a precise infusion rate.
