Carbon is oxidized from the -2 to the +2 state. Iodine is reduced from the zero state to the -1 state
Since carbon is being oxidized, then iodine is reduced, and iodine can only be reduced to -1. 

The problem is that carbon is oxidized from -2 to +2 in both CHI3 and HCOO-. 
The final equation is ... 

C2H5OH + 4I2 + 6OH- --> CHI3 + HCOO- + 5I- + 5H2O

I am balancing by oxidation number method. In carbon n factor will be change in oxidation state multiply atomicity so 4*2=8 and for iodine n factor will be 1*2=2. 
So ratio= 4:1 so coefficients of iodine will be 4 and carbon will be 1 in left side. Then we balance iodine and charge
