What is the format of disability certificate to be taken by the pwd students during iit mains entrance exam rather than admit card?

dear princy
you should not carry that to the exam center , in exam center you have to carry only your aadhar card and hall ticket printout .
and after you qualify mains and appear for advanced or during mains councelling you have to produce disability certificate .
