I am presently in class 12 and preparing for JEE... I haven't done any hard studies in both the years of preparation only moderate level of questions... if I put in my complete efforts in these 2 months do I have a chance of getting AIR 5000 approximately in JEE Advanced ??

ya , for sure but u need to do smart work and smart work means u need to put every time to study . 
1- make a time table and stick to it , but remember – make short time table like that of next 5 days , and try to complete it 
2-take previous year ques , they are the only thing that can benifit you now . 
3- take some imp topics and revise that now . 
u can enroll to our crash course , they will be ebnificial to many students like you
