Hence
 
V =-E.x
 
This is the potential at positive x- axis.
Yes you have to consider the negative sign.
Option B is correct.

Clearly E= - V/d.
Now V is - Ed.
Here d is x .
Hence V= - Ex.
See they have not asked magnitude but the value.
If magnitude was asked the answer would be option a).
Hope it clears it doubt.
