Dear student,

As soon as plates are connected by a conducting wire, the system of the two plates and the wire become a single conductor. Now we know that electric field inside a conductor is zero and so the potential at all points inside it is same. Therefore the charges will rearrange on their surfaces to finally make the plates attain equal potential.
