The increase in the girth of a stem or secondary growth takes place due to the presence of lateral meristem, cork cambium, and vascular cambium. Apical meristems are found on the apices / growing parts of a plant such as tips of shoots, roots, etc. Intercalary meristems are located in internodes, the base of the grass and between leaves. 
So, the correct answer is option B.
