D. When the distance of the object from the mirror is equal to the focal length of the mirror.

In a convex mirror, the image formed is always virtual, diminished, and upright. The image appears to be located behind the mirror. When the object is placed at a distance equal to the focal length of the convex mirror, the virtual image forms at infinity, and it can be seen at this point. At other distances, the image may not be visible or may be seen only partially.
